Foley raised concerns about a delay in providing full CCTV footage of the alleged attack owing to the premises supervisor not being on site.

“It delayed and hindered a live murder investigation,” he said.

Foley said the combined problems identified amounted to “fundamental failures of management, supervision and leadership that contributed to an environment in which serious violence occurred and where the emergency response was subsequently hampered”.

The bar’s licence had been temporarily suspended in the wake of the incident but councillors decided to further suspend it for three months, allowing for a “full reset” of the business.

As part of the new set of licence conditions before it can re-open, the bar must also stop serving alcohol at 01:00, change its designated premises supervisor, introduce mandatory searching of customers and implement CCTV improvements.

In a tribute after his death, Cullen’s mother said: “Shane was a big, gentle giant who wouldn’t hurt anyone.

“He was a good dad, son and brother. He will be sadly missed.”

Carl Cooper, 36, of Smallshaw Lane in Ashton-under-Lyne, has been charged with Cullen’s murder and is due to stand trial in January next year.
