The Met, British Transport Police (BTP) and Crown Prosecution Service (CPS) have all admitted failures in how Levy was monitored between his first conviction in September 2021 on sexual assault charges and his arrest for murder in September 2025.
On Saturday, Simons told Breakfast a “multi-agency review” was needed, saying: “There was a lack of restrictions that could have been placed on him and a failure to monitor him appropriately – so all of those things, I think, we need answers to now.”
Simons said public services “have to get better at recognising patterns of offending” before attackers become “emboldened”.
The Met said its detectives had “exhausted numerous lines of enquiry – including working with coroners – to identify whether Levy is responsible for any similar, unsolved incidents, including deaths”.
No further victims have been identified so far but the force urged “anyone who believes they may be a victim of his to contact police”.
Levy is known to have attacked at least 10 women on public transport in London, seeking out busy carriages to assault passengers.
BTP said more women “may recognise [Levy] and have not yet reported what has happened to them”.
